We recommend oral antiviral therapy for immunocompetent children and adolescents at increased risk of complications from varicella (e.g., pneumonia, skin infection) [14-16]. This therapy may reduce the risk of complications and is generally well tolerated. At-risk groups include:

- Unvaccinated adolescents (≥ 13 years old), who are more likely to experience severe disease than younger children [17].

- Secondary cases among household contacts, which tend to be more severe than primary cases [18].

- Patients with chronic skin or lung conditions, as they are at higher risk for severe secondary bacterial infections.

- Children using intermittent oral or inhaled steroids, especially during the incubation period.

- Individuals on chronic salicylates, who are at risk for Reye syndrome (see "Acute toxic-metabolic encephalopathy in children," section on 'Reye syndrome').

We suggest oral antiviral therapy (valacyclovir, acyclovir, famciclovir) for immunocompetent adults with uncomplicated variϲеllа infection. Although most adults who develop vаricella have an uncomplicated course, they are at increased risk of developing pneumonia, which often leads to hospitalization and carries an overall mortality of between 10 and 30 percent
