과민성 폐장염의 확진검사는 없습니다. 다만 병력과 여러 검사소견을 종합해 진단을 냅니다.

1) Exposure to Ag, neutrophilia, lymphopenia(eosinophilia 는 아님)

2) CRP, RF, ESR, immunoglobulin

3) 원인 Ag에 대한 특이 침강 항체. 이 자체로 확진을 할 수는 없으나， 진단에 중요한 단서!!!

4) Chest X-ray : ILD pattern, nonspecufic

5) CT : procedure of choice for HP

(1) 급성 시 GGO in lower lung, centrilobular nodule

(2) 만성 시 interstitial fibrosis 와 비슷한 양상

(honeycombing 등) bilateral GGO & centrilobular emphysema

• Hilar or mediastinal lymρhadenopathy 는 관련이 없다

6) PFT

(1 )문제엔 주로 restrictive로 주어집니다. or obstructive pattem

(2) Lung volume 감소

(3) Impaired diffusion capacity

(4) Decreased compliance

(5) Exercise induced hypoxemia

• 항원을 피하면 PFT 상의 비정상은 reversible하지만 노출 후에 점점 악화될 수 있다.

7) BAL : lymphocyte dominant( CD4/CD8 림프구 비율 <1)
