AVNRT에서 impulse가 “한 지점에서 두 갈래로 동시에 완전히 분기한다”기보다는, AV node 주변의 reentry circuit을 돌면서 각각 ventricle로 나가는 exit와 atrium으로 올라가는 retrograde limb을 통해 activation이 전달된다고 보는 게 더 정확합니다.

특히 typical slow-fast AVNRT에서는 impulse가 slow pathway를 따라 내려와 lower common pathway/His bundle 쪽에 도달하면 곧바로 His-Purkinje system을 통해 ventricle이 빠르게 depolarization되어 narrow QRS가 생깁니다. 한편 retrograde atrial activation은 fast pathway를 통해 atrium 쪽으로 올라가야 하므로, atrial depolarization은 QRS와 거의 겹치거나 QRS 말단/직후에 나타납니다.

그래서 실제로는 시간 차이가 아주 짧아서 거의 동시에 일어나는 것에 가깝지만, ECG에서는 ventricular depolarization의 전기 신호가 훨씬 커서 retrograde P wave가 QRS 안에 묻히거나, QRS 직후에 pseudo r′ in V1 또는 pseudo S wave in inferior leads처럼 보입니다.

즉 “동시에 안 일어나는 것”이라기보다는, conduction time 차이 + QRS가 P wave를 가리는 효과 때문에 surface ECG에서 순차적으로 보이는 것입니다.

윗 분 말대로 QRS에 묻혀서 안 보이는 경우가 제법 많습니다.

In typical forms, the conduction time from the compact AV node region to the atrium is similar to that from the compact node to the His bundle and ventricles, such that atrial activation occurs at about the same time as ventricular activation. The P wave is therefore inscribed during, slightly before, or slightly after the QRS and can be difficult to discern. Often the P wave is seen at the end of the QRS complex as a pseudo-r′ in lead V1 and pseudo-S waves in leads II, III, and aVF (Fig. 256-2).

More unusual forms of AVNRT have P waves falling later, anywhere between QRS complexes, in which case, an inverted P wave is seen in the inferior limb leads with the inverted P wave seen in the subsequent T wave.
