Amiodarone, a class III drug, appears safe for patients with preexcited AF based on a systematic review of 12 studies that included two populations: patients with preexcited AF who received IV amiodarone and patients with overt or concealed accessory pathways who underwent electrophysiology study (EPS) and received protocolized IV amiodarone with measurement of the anterograde effective refractory period (AERP) of the accessory pathway before and after infusion [22]. Of 146 patients who received amiodarone, none experienced rate acceleration or ventricular fibrillation. In the three studies in which patients underwent EPS, the AERP of both the AV node and the accessory pathway increased following amiodarone infusion.
