• 가임력 보존하는 경우 = 자궁체부 보존
원뿔절제술, 근치자궁경부절제술
• 가임력 보존하지 않아도 되는 경우 = 자궁체부 절제
Type A: 단순자궁절제술
Type B: 변형근치자궁절제술
Type C1: 근치자궁절제술
• conservative surgery criteria
- Cone biopsy (preferred)
- No LVSI (preferred)
- Negative cone margins for cancer (preferred)
- Squamous cell (any grade) or usual type adenocarcinoma (grade 1 or 2) (preferred)
- Tumor size ≤2 cm
- Depth of invasion <10 mm on cone. If no conization, MRI must show <50% cervical stromal invasion
- Negative imaging for metastatic disease (MRI recommended)
<Category1에 해당하는 치료>
(IA1, LVSI(-) 제외하고 전부 PLND 같이 시행)
IA1, IA2, IB1 (2cm 이하)
criteria 만족
가임력 보존O: 원뿔절제술
가임력 보존X: 단순자궁절제술 (type A)
criteria 만족하지 않는 IB1
가임력 보존O: 근치자궁경부절제술
가임력 보존X: 근치자궁절제술 (type C1)
IB2, IIA1 (4cm 이하)
가임력 보존X: 근치자궁절제술 (type C1)
IB3, IIA2
