Dexamethasone is the glucocorticoid agent most commonly used in the prevention and treatment of BРD. Although randomized clinical trials have shown that postnatal ԁехаmеthаѕоnе therapy reduces the risk of BΡD, it is associated with serious adverse effects, including СP, especially when high doses are used within the first week after birth. Thus, we continue to recommend against routine use of early ԁехаmеthаѕοne therapy. However, the benefit-risk ratio depends on the underlying risk of ΒРD. In high-risk patients (ie, older infants with evolving or established ΒРD who remain ventilator-dependent after two to four weeks), the benefits of low-dose ԁехаmеthаѕоnе likely outweigh the risk of adverse effects.

알렌 해설에서도 말하듯이 폐표면활성제는 출생 시에 이미 줬을 것 같고... 지금 인공호흡기 치료를 4주 이상 지속하고 있는 상황이니 low-dose dexamethasone을 DART protocol에 따라 IV로 투여하는 것이 좋겠습니다.
